Shoulder situation remains murky
Carr (shoulder) has been to New Orleans twice in recent weeks to meet with the Saints' team doctor, Jane Slater of NFL Network reports. Carr ended the 2025 season on the sidelines with a broken left hand and an oblique injury that he suffered in Week 14 against the Giants, but it has now come to light that his current shoulder issue stems from that same game. The quarterback apparently didn't notice the injury to his throwing shoulder when he was shut down from throwing while recovering from the aforementioned injuries. Once he began ramping up for the team's spring program, he noticed the shoulder issue. Carr has yet to make a decision on if he'll undergo surgery on the ailing shoulder and his availability for the 2025 campaign remains unclear, but a decision is expected to be made in the near future.
New Orleans awaiting clarity
Saints general manager Mickey Loomis said Wednesday that Carr "does have an issue with his shoulder" and that the team is "hoping to get some resolution and clarity on that in the near future," Katherine Terrell of ESPN reports. Loomis outwardly declined to answer any questions about Carr's injury or status with the team at Wednesday's press conference, additionally refusing to answer affirmatively or negatively when asked whether New Orleans' starting quarterback is currently on the roster, per John Sigler of USA Today. Carr is reportedly considering shoulder surgery that could impact his availability for 2025 due to a previously undisclosed injury, with speculation that the decision is related to dissatisfaction with the Saints and a desire to move on. Due to the veteran quarterback's contract being restructured in March, he is owed roughly $40 million guaranteed in salary and bonuses this season.
Wants out of New Orleans?
Carr's discussion of shoulder surgery may be related to dissatisfaction with the Saints and a desire to be released, according to Mike Triplett and Nick Underhill of NewOrleans.Football. Carr reportedly preferred to be released earlier this offseason, but the Saints instead did a simple contract restructure to shift more of his cap hit to future seasons without adding/removing money. It was then revealed last week that the 34-year-old QB is considering surgery for a previously unreported shoulder injury, and with a recovery timeline that could keep him out for the start of the 2025 season. It's hard to say exactly what's going on here, but it is clear that fantasy managers shouldn't assume Carr will be the Week 1 starter in New Orleans under new head coach Kellen Moore. The other options under contract are Spencer Rattler, Jake Haener and Ben DiNucci, which makes the Saints a top candidate to draft a QB on Day 1 or 2 next week.
May require shoulder surgery

Carr, who missed seven games in 2024 (including the Saints' final four contests) due to a left oblique injury and a fractured left hand, is now dealing with an injury that could significantly impact the Saints' coming season, as well as the team's decisions in April's draft. Further details regarding the veteran signal-caller's injury should be forthcoming, but Rapoport reports that surgery is among the options being considered by Carr and New Orleans. With only Spencer Rattler, Jake Haener and Ben DiNucci currently available behind Carr on the depth chart, the Saints figure to be increasingly motivated to explore remaining free agency options at quarterback, and the team's decision at No. 9 overall in the 2025 NFL Draft could now more realistically trend toward the position.
Restructuring contract with NOLA

Carr missed seven games in 2024 due to a left oblique injury and a fractured left hand, and he finished the regular season completing 67.7 percent of his passes for 2,145 yards, 15 touchdowns and five interceptions across 10 games.
Viewed as starter
Saints general manager Mickey Loomis said Wednesday that he views Carr (hand) as the team's starting quarterback heading into the 2025 season, Katherine Terrell of ESPN.com reports. Loomis said that he believes the Saints "can win with" Carr, echoing the tone of new head coach Kellen Moore, who said earlier this month that the 33-year-old is a "tremendous starting quarterback in this league." Regardless of New Orleans' enthusiasm, Carr's $51.4 million cap hit for 2025 poses a significant hurdle that the team will have to address in order to maneuver properly in free agency. Terrell reports that restructuring Carr's contract could free up $31 million in cap space for the 2025 season, at the cost of making his cap hit in 2026 -- the final year of his contract -- roughly $69 million. Carr is recovering from an injury-riddled campaign in which he most notably missed the final four games of the season due to a broken bone in his non-throwing hand.
Offseason cut candidate?

They can release him before mid-March with a post-June 1 designation to push most of the dead-money charge to 2026, but that wouldn't free up 2025 cap space until June (even if the transaction is made long before). Carr will turn 34 in March, following a 2025 campaign in which he suffered numerous injuries and eventually missed the final four weeks with a broken bone in his non-throwing hand.
Ruled out for Week 18
Carr (hand) has been ruled out for Sunday's game against Tampa Bay, Nick Underhill of NewOrleans.Football reports. Reports suggested Carr was unlikely to play again this season after he suffered a concussion and broken left hand during a Week 14 win over the Giants. Interim coach Darren Rizzi decided to treat it as a week-to-week matter, at least publicly, but Carr never even returned to practice. Spencer Rattler will make another start Sunday, with Carr then having plenty of time to recover this winter and get back to full strength before OTAs. Carr doesn't currently have any guaranteed money for the final two seasons of his four-year contract, but it'll be difficult for the Saints to move on and absorb a huge dead cap hit given their unusual salary-cap situation/management. Carr's $30 million base salary and $10 million roster bonus for 2025 both become guaranteed in mid-March, which means there will be some kind of renegotiation/restructure before then even if the Saints plan to keep him around as their starting QB for 2025. Carr will turn 34 shortly thereafter, at the end of March.
